Why are you asking for my personal contact details?

We need to know your full name and have a current email address and phone number for you as we will need to contact you if the individual you have nominated is successful in being shortlisted for an award. Incorrect information will make getting in touch with you very difficult and may affect the chances of your nominee going any further in the selection process

You will not be able to progress through the form unless you have entered a valid email address and phone number. 

Why are you asking for contact details of the person I am nominating? 

We require the contact information for the person(s) you are nominating so we can let them know if they have been shortlisted for an award.  The person(s) you are nominating will receive an email telling them about your nomination.  You may want to let them know beforehand that you have submitted a nomination for them.
